Prioritize Care, Prevent Hospitalizations
Mrs. Alvarez wasn’t just another patient—she was a puzzle the system couldn’t solve. Despite having all the right data, her care team lacked a simple way to flag her as high-risk. Dr. Katz, exhausted by preventable hospital visits, imagined a solution as effortless as liking a song on Spotify.
That’s when we built Case Collaboration, a feature that let providers instantly mark high-risk patients.
The results? A 35% drop in preventable admissions, more revenue, and better patient outcomes.

How can we better track high-risk patients, reducing hospital admissions?
PROBLEM TO SOLVE
Providers need a simpler way to monitor high-risk patients without adding paperwork or stress. Without this, some at-risk patients may be overlooked, leading to unnecessary hospitalizations.
GOAL
The goal is to create a tool that simplifies tracking and integrates seamlessly into existing workflows to reduce hospital admissions.

What I’ve Learned
The Good
Strong research + storytelling; including stakeholders in the design process
Working in-house made it easy to connect with users for research. Involving stakeholders and addressing their needs directly helped me sell my design effectively, making them feel heard and understood.
The Bad
The final filter modes takes a few steps and clicks to access
If I had more time, I would want to make the filter easier to access (less clicks). But it would mean changing it from the ground up, which was a larger scope than what we had the resources for at the time.
Opportunities
Always do own research regardless of how much research has already been done
Sometimes it’s easy to breeze over the research portion because previous research has already been conducted, however, the success of this project was due to conducting my own research. If I didn’t insist on doing this, I wouldn’t have had as successful of a product.
